Supporting evidence you need to provide with a disabled person's bus pass application

This page describes the documents you'll need to provide for each category.

Blue Badge

We can check if you have a Blue Bage on our system. If you permit us to check, you do not need to upload a photo of your valid Blue Badge.

You do not have to permit us to check. In that case, you need to upload a photo of your valid Blue Badge.

Disability Living Allowance mobility component

Checking with the Department for Work and Pensions (DWP)

We can check your entitlement with the DWP. If you provide your National Insurance number and permit us to check your benefit, you do not need to upload your letter from the DWP.

Upload your letter

You do not have to permit us to check with the DWP, but we do need to see proof of your benefit. So, you must provide a photo or scanned image of your most recent Disability Living Allowance uprating letter (dated within the last 12 months).

When you upload your letter, it must clearly show:

  • your name and address
  • the date of the letter
  • that you receive the highest rate for the mobility component
  • the expiry date of the award.

Personal Independence Payment mobility component

You need to provide a photo or scanned image of your most recent letter from the Department for Work and Pensions (dated within the last 12 months).

When you upload your letter, it must clearly show:

  • your name and address
  • the date of the letter
  • the breakdown of points confirming you receive 'Mobility - Moving around - 8 points (or more)' or 'Daily living - Communicating verbally - 8 points (or more)'
  • the expiry date of the award.

War Pensioner's mobility supplement

You can prove your eligibility by providing the following:

We need to see your letter from Veterans UK. You should receive your letter in March confirming your payments from April. We will need to see the award letter if you have only recently been awarded the supplement.

Blind or partially sighted

You can prove your eligibility by providing one of the following.

  • Certificate of visual impairment (CVI) completed by an ophthalmologist.
  • Form BD8 (issued from 1990, but replaced by the CVI).
  • Evidence of registration with the local authority as being blind (severely sight impaired) or partially sighted (sight impaired). In Oxfordshire, the register is managed by Oxfordshire Visual Impairment Team.
  • For veterans, the Reasons for Decision’ letter under the Armed Forces Compensation Scheme (AFCAS).

Profoundly or severely deaf

You can prove your eligibility by providing one of the following.

  • Confirmation that you are registered as profoundly or severely deaf from a local authority.
  • Letter from an audiologist or audiological report.
  • Documents which demonstrate that you have hearing loss of more than 70 dBHL in both ears. These may include letters from consultants or GPs.
  • For veterans, the Reasons for Decision’ letter under the Armed Forces Compensation Scheme (AFCAS).

Without speech

You can prove your eligibility by providing one of the following.

  • Evidence that you have been awarded 8 points or more for the ‘Communicating Verbally’ activity of Personal Independence Payment (PIP)
  • Letter from Social Services or another appropriate organisation providing support services, stating you are without speech.
  • Documents that demonstrate you are without speech. These may include letters from your consultant or GP.
  • For a child, an Education Health Care Plan (EHCP) or Statement Educational Needs (SEN) that demonstrates they are without speech.

Unable to walk, or considerable difficulty when walking

You can prove your eligibility if:

  • you provide a letter from a specialist describing your disability, if it is permanent or temporary, the expected duration and that you cannot walk more than 64 metres without severe discomfort
  • you have a valid Blue Badge
  • you get the Personal Independence Payment mobility component 
  • you get War Pensioner's mobility supplement.

Without arms or have loss of use of both arms

You can prove your eligibility by providing one of the following.

  • Documents which demonstrate that you have lost the use of your arms over the long term (12 months or more). These may include letters from a medical professional or consultant.
  • Letter from a medical practitioner who is treating you.
  • For veterans, the Reasons for Decision’ letter under the Armed Forces Compensation Scheme (AFCAS).

Living with a learning disability

You can prove your eligibility by providing information which confirms you have both:

  • significant impairment of your intelligence
  • significant impairment of your social functioning.

We may ask you to send in additional information such as:

  • Statement of Educational Health Care Plan (EHCP)
  • Special Educational Needs (SEN).

Unfit to drive because of a medical condition

You need to show that one of the following applies:

  • Your driving licence has been revoked on medical grounds.
  • You have been refused a driving licence on medical grounds.
  • You would be ineligible for a driving licence on medical grounds.

Any evidence you provide should be dated within the last 12 months.

You can prove your eligibility by providing one of the following.

  • Letter from the DVLA indicating that your driving licence has been refused or revoked following an assessment.
  • Letter from the DVLA accepting a surrendered driving licence, together with evidence of the relevant medical condition.
  • Information from a medical professional that confirms you would be refused a driving licence (other than because of alcohol/drug dependency).
  • Evidence which demonstrates you have one of the disabilities or medical conditions which would be covered by Section 92 of the Road Traffic Act 1988 (as listed in paragraphs i. to v.). This may include letters from a medical professional or consultant.
