What you can expect after your needs assessment with us.

What happens at the needs assessment

At your  needs assessment we will talk together about:

  • What is and who are important to you
  • What is preventing you from doing what you would like to do or seeing who you would like to see
  • how you want your life to be
  • what you are having difficulty with
  • what support you have and what support might enable you to live more independently 
  • We will also talk to your partner, relatives and friends if you want us to. Through the assessment we can build up a complete picture of you, what’s important to you and the different aspects of your life. Let us know if an interpreter or advocate is needed. 

What is a support plan?

After your needs assessment, if you are eligible for our support, we will work with you to develop a support plan. 

Your support plan will outline what you want to achieve and how you want your needs to be met. To make sure that you get the right support, we may talk to other health or social care professionals, but we’ll always ask your permission before we talk to them. We know things change, so your support plan will be reviewed on a regular basis.

We will then start working closely with you to put in place the services that will support you. 

If I’m not eligible

If you’re not eligible for support from us, there is lots of local help that you can get to help with your care.

About direct payments

In some cases, rather than provide the support service, we may provide you with a direct payment. Direct payments give you choice and control over how your needs are met.

How will my care be paid for

If your needs assessments indicates you are eligible for support from us, you will need to complete a Financial Assessment to determine whether you need to contribute to the cost of your care. Find out more about the different ways you can pay for your care.
